An update on an incident involving one of our systems

As of 12:00 pm ET, October 22, 2026

We have identified an incident involving unauthorized access to our Quanum Lab Services Manager (LSM) system, which certain clients use to place test orders, receive and look-up results. Upon discovery of the incident, we paused access to the system and we are continuing to investigate the incident. Our core business operations are not impacted and remain up and running.

Providers that use direct-customer LIS interfaces or EMRs to order testing from us may continue to place orders through those interfaces, which were not affected.

We take privacy and security very seriously and are committed to full restoration of our systems as soon as possible.

While no data was accessed in these other systems, out of an abundance of caution we have paused access temporarily.

Once we confirmed the unauthorized access, we quickly paused access to the Quanum LSM and there is no more access to the system. We are continuing to investigate this incident and are working to fully restore the affected system as quickly as possible.

Yes, we are communicating with the appropriate authorities in law enforcement.

We believe some patient data within Quanum LSM was accessed, but other systems were not affected. We are working diligently to investigate the incident to determine the specific data affected.  As the covered entity, Quest Diagnostics will make notifications as required by applicable law.

Yes, but we are still investigating to determine the specific data affected.

Core business operations are not impacted and remain up and running.  Providers that use direct-customer LIS interfaces or EMRs to order testing from us may continue to place orders through those interfaces, which were not affected.  Quanum LSM customers may currently utilize paper requisitions or Quanum eLabs to place orders and receive results. We anticipate restoring full-service functionality as soon as tomorrow, October 23.

While we are still investigating the extent of the prior unauthorized access to Quanum LSM, the access has been discontinued. Our core business operations are not impacted and remain up and running. Providers that use direct-customer LIS interfaces or EMRs to order testing from us may continue to place orders through those interfaces, which were not affected.

We regret the inconvenience to you, your staff and your patients.  For the time being, clients can use paper requisitions to place orders for testing. You may submit these requisitions via fax, please use our Location Finder  to find your local PSC(s) then click on the preferred site to find our fax number.  To be clear, providers that use their EMR to place orders should continue to order from us as usual. To request results by phone or eFax, you may contact Quest National Customer Services at 1-866-MYQUEST.

Quanum LSM clients may also utilize Quanum eLabs to place orders and receive results. Quanum LSM is currently redirecting clients to eLabs where they can use the same credentials to log in. Instructions on placing orders and receiving results in eLabs are available at the site. Support with eLabs is available at the Connectivity Help Desk at 1-800-697-9302.

Please contact National Customer Services at 1-866-MYQUEST or use eFax.

We do not expect delays for clients that have not used Quanum LSM. Our core business operations are not impacted and remain up and running.  Providers that use direct-customer LIS interfaces or EMRs to order testing from us may continue to place orders through those interfaces, which were not affected.

We are working diligently to identify the impacted patients and providers. As the covered entity, Quest Diagnostics will make notifications as required by applicable law.

Based on our investigation, the incident is not related to a PHP programming language issue.
